When caring for someone with dementia, it’s important to understand their needs, preferences, and personal history. That’s where a 'this is me' document comes in. These documents help carers provide the best possible care by knowing more about the person beyond their medical condition.
A 'this is me' document is a simple form filled out by or for a person with dementia. It contains key information about their personal likes, dislikes, routines, background, and important details that make them who they are. It is designed to help carers understand more about the individual, making sure their care is tailored to meet their unique needs.
This document can be especially useful when a person with dementia moves into a care home, goes into hospital, or starts receiving care from someone new. It’s a way to keep a person’s life story and preferences at the centre of their care.
The 'this is me' document typically includes:
Personal details: name, age, and preferred name or nickname.
Background and life history: where they grew up, important family members, work history, hobbies, and interests.
Likes and dislikes: favourite foods, music, activities, or things they enjoy. It can also include things they dislike, such as foods they won’t eat or things that might upset them.
Daily routines: how they like to start their day, when they usually eat, nap, or sleep, and any specific routines that make them comfortable.
Communication preferences: whether they have hearing or sight issues, how they prefer to communicate, or if they respond better to certain tones or phrases.
Health conditions: any other health issues they have, like diabetes or allergies, along with details on medication if needed.
Things that help them feel safe and calm: special objects, music, or routines that help reduce anxiety.
Family and friends: key people in their life and how they prefer to stay in touch.
A 'this is me' document is shared with carers, doctors, nurses, or anyone involved in the person’s care. It helps them provide care that is personalised and respectful of the person’s preferences.
For example:
In a hospital setting, where staff may not know the patient well, the document can help them understand important details quickly. Knowing how the person likes to communicate or what might trigger anxiety can make a big difference.
In a care home, it helps staff create a familiar and comforting environment by following the person’s daily routines and engaging them in activities they enjoy.
At home, family carers and support workers can use the document to ensure the person’s needs are met in a way that respects their individuality.
People with dementia might struggle to communicate or remember things about themselves, especially in new or stressful situations. A 'this is me' document allows their carers to know and respect the person’s identity, even if they cannot express it themselves. It’s about treating the person, not just the condition.
